Clinical Case Management Services in California
The West Coast Counseling Center team has found that crisis in emotional health or addiction may require more time beyond the therapy hour and a more complex system of treatment.
Phase One of Clinical Case Management services require a higher level of care and supervision. Our goal is to identify and coordinate individualized services and to create a treatment team of professionals. The team will focus on helping clients find stability, autonomy, and balance in their daily lives. Clinical Management therapists may need to have more extensive communication with family members, partners, adjunctive providers, and other professionals.
Phase Two goal is to see the level of risk decrease and the level of responsibility and independence increase with time. There will also be less need for additional services, supervision, or contact with treatment team providers. Clients at this stage will begin to feel more stability as they depend on their own resources, identify meaningful relationships, and develop a supportive network.
